HumanitasCare Essential

Designed to connect you with experienced and available service providers (SLP, OT, PT, etc) to help manage your child's treatment plan.

A Humanitas liaison is responsible for the following:

  • Connecting you with service providers that fit your child's unique needs.
  • Confirming their availability, coordinating the frequency of services, and scheduling sessions.
  • Conducting a review of your child's progress as it relates to the overall treatment plan.
  • Providing you with ONE platform where you can see and manage ALL upcoming appointments and payments.

    HumanitasCare PLUS

    Designed to provide an added level of support in the treatment management process.

    In addition to the services outlined under the HumanitasCare Essential package, the liaison is also responsible for the following:

  • Direct communication with your child's health care and service providers.

  • Arranging and scheduling transportation to and from health care and service provider appointments.

  • Securing support staff to stay with your child throughout treatment sessions.
